Reservation Forms
To expedite your reservation request, please follow these instructions:
1. Download the appropriate Reservation Request Form, Policies Agreement and, if appropriate, Acknowledgement of Risks Form below. These forms must be returned to complete your registration.
2. Fill out each form as completely as possible.
3. Review carefully before signing each form and fax to 435-645-6937.

Reservations - Advance reservations are essential for all Deer Valley services throughout the ski season, especially during the December holiday, Martin Luther King weekend, Presidents' Day week and the month of March. Phone reservations are available daily from 8 a.m. to 5 p.m. MST by calling 888-754-8477 or 435-645-6648. All products are subject to availability and may sell out at any time. It is highly recommended that only one family be included on each reservation.
Payment - The full cost of Skier Services products reserved in advance will be charged to your credit card at the time of booking.
Cancellations/Changes - We will gladly give a full refund if reservation is cancelled before 5 p.m. two days prior to the reservation; otherwise, the following fees will apply:
Private Lesson - $100
Max 4, Teen Escape, Reindeer Club, Adventure Club, Bambi and Fawn - $75
Child Care - $45
Ski Rental - $20
Specialty Programs - $75*
*Please note: Specialty Programs must cancel before 5 p.m. two days prior to the first day of the program, not during the program itself.
Product Transfers - Skier Serices products are not transferable to another person, date or program.
No-shows - Full-day rate will be charged for the missed day and remaining days will be cancelled. Products cancelled after the start time are considered a 'no-show.
