Camp Reservation Forms

To make a reservation for Summer Adventure Camp, please follow these instructions:
1. Download both of the forms below.
2. Fill out each form as completely as possible.
3. Review each form carefully before signing.
4. Please fax completed forms to 435-645-6937 or email to childara@deervalley.com.

Due to the popularity of this program, we often reach camper capacity. Advance reservations are essential. Please call 435-645-6648 or 888-754-8477 for availability, reservations or additional information.

Cancellations/changes must be made before 5 p.m. two days prior to the reservation; cancellations for Monday or Tuesday must be made by Friday; otherwise a cancellation fee of $25 will be charged.
